Community Health Improvement Priorities Survey

We want to hear from you!

Several health-focused organizations in Mesa County are beginning a process of planning collaborative projects to improve health and quality of life in our community. Today we’re interested in which areas you would like to see multi-organization, cross-sector focus on in the next two to three years.

Every three years, health focused organizations in Mesa County create a Community Health Needs Assessment to understand the health outcomes, health decisions, and factors that impact health decisions for Mesa County residents.

These organizations include:

  • Mesa County Public Health
  • St. Mary’s Hospital
  • Community Hospital
  • Monument Health
  • Family Health West Hospital
  • MarillacHealth

The 2024 assessment identified eight areas of action for these organizations and their community partners to focus on. Most organizations will choose one to four priority areas and work together to design effective programs to improve health in our community. Each individual organization will also have other priority work areas, some from this list, some from other sources.

You'll find the eight identified priorities on the next page. Review them, and click "next" to rank the priorities to show your support.
